Case Management and Comprehensive Support
Our Care Support Specialists (CSS) play a pivotal role in guiding neighbors through the steps of housing navigation and work to support each neighbor to achieve stability. With a caseload limited to 30 households, each CSS is able to dedicate the time and attention necessary to fully support the neighbors they work with. This includes accompanying them to appointments, advocating on their behalf, and coordinating with other service providers to ensure a comprehensive, wraparound approach.
Case Management
We meet each neighbor where they are, building trust and creating individualized plans to help them reach stability. CSS assists with:
Housing applications
Gathering important documents
Scheduling and attending appointments
Connecting neighbors to medical, legal, and mental health services
Collaboration
Healing happens in community. Our CSS team works closely with other service providers, healthcare partners, landlords, and community organizations to ensure neighbors have comprehensive, wraparound support. We advocate on behalf of our neighbors and help coordinate services that promote long-term stability.
CalAIM Services
Through CalAIM (California Advancing and Innovating Medi-Cal), we provide enhanced care coordination for neighbors with complex health needs.
Helping neighbors access healthcare services
Providing transportation to medical appointments
Advocating for neighbors within the healthcare system
Offering ongoing support for long-term health and stability
Peer Support
Neighbors supporting neighbors is at the heart of what we do. Many of our team members have lived experience with homelessness, offering a unique perspective and deep empathy. This connection helps build trust and creates a supportive environment where neighbors feel seen, heard, and encouraged on their journey.
